How Do I Make A Family Status Change?

There are three situations under which you will be able to change your benefit coverage choices during the year:

  • If you have a change in status.
  • If you experience another event, such as a change in your spouse's plan, under which other election changes are allowed.
  • If you had not enrolled yourself or your dependents due to other medical coverage but lose that other coverage.

Refer to the General Information Summary Plan Description located on the Human Resources website for details on qualified changes in status.

The following steps outline how to make a change in family status.

Step 1 - Contact the Human Resources Benefits Office

  • Call the Benefits Office at extension 28232
  • Inform Benefits of a Family Status Change
  • Request a Flexible Benefits Enrollment-Family Status Change form

Step 2 - Complete Family Status Change Form

  • Complete the Flexible Benefits Enrollment-Family Status Change form
    • Make changes to benefit plans consistent with your change in status
    • Include spouse/dependent information if necessary
    • Include beneficiary information if necessary
  • Attach documentation related to your change in status
  • Return the completed form and accompanying documentation to the Benefits Office

Additional Steps

  • Anticipate new medical and/or dental insurance cards if applicable within 7-10 business days from Human Resources receiving your completed form and supporting documentation

Changes become effective as of the date of the change in status. If you do not implement a change in status within 31 days of your change in status or other event, you will not be allowed to change your coverage until the next annual enrollment.

Any change in your deductions (if applicable) will be effective the first pay period following the change.
